Re-engaging cold lists ("we haven't emailed you in 12 months, let's see"). Buying email lists. Aggressive subject line testing without monitoring complaint rates. Using a single domain for marketing + transactional email. Failing to authenticate with DMARC.

Use mxtoolbox.com to check SPF, DKIM, DMARC. Check Gmail Postmaster Tools for your domain reputation. Verify you have list-unsubscribe header. Look at last 30 days of campaigns: complaint rate, bounce rate, open rate trend. If any number is sliding, you have a deliverability problem.
Recovery if you're throttled
It takes 4-6 weeks to recover sender reputation. Reduce send frequency. Email only highly engaged subscribers (last 30-60 days open). Fix all authentication. Improve unsubscribe ease. Don't panic-send to "win back", that makes it worse.
